• Proof of Income, if applying for the National Nutritional School Program
• Valid Identification of Person Enrolling Student (Driver's License,Social Security Card, Military ID)
• Proof of Residence (Utility Bill, Lease)

• FEWER THAN FIVE COURSES (PER COURSE)...$149.00.
