Required Forms

Birth Certificate (a certified copy) A Baptismal Certificate or hospital record may be used in lieu of a birth certificate provided a certified copy is presented within thirty days
Health and Immunization Records
Report card/transcript from last school attended
Social Security Card
Proof of Income, if applying for the National Nutritional School Program
Valid Identification of the parent/guardian enrolling the student (Driver’s license, Social Security Card, Passport)
Proof of Residence (Utility Bill, Lease)
Students who are under 18 years of age and do not reside with a parent must complete a responsible person affidavit.

County Transfers, Grades 1 – 12	$475.00
