Dear Parents and Guardians,
We want to inform you that an Opt-Out form will be sent home with your child for a social-emotional growth measure survey that will be administered to students in grades 3, 4, and 5 at the beginning, middle, and end of the school year. This survey is designed to assess students' social and emotional development and help us support their growth. Participation in the survey is voluntary, and you may choose to opt your child out by submitting the completed form.
